Try Diagnostic Instruments, they list adapters for a bunch of Polyvars.
I've used them in the past and they are excellent. (www.diaginc.com)

That's a very nice collection of adapters you have listed on your web page.

Do you happen to have a male T-thread to female C-thread adapter? I have
found the female T-thread to male C-thread adapter (like the one listed from
Edmunds Optics at [1]) but not the opposite.

Edmund has a male T-to-male T ring, this will solve your problem. See

Stacks of off-the-shelf parts work if the extra length tolerable. A custom
adapter is needed if the length is to be minimized.

Yes. Try Diagnostic Instrument Inc. I got their 1x C-mount for a 1/2" CCD.
They wrote a useful document about microscope couplers:
